I’m an In-Network provider with Premera, LifeWise, Kaiser Permanente, and First Choice Health Network.

For those who wish to access out-of-network benefits, I can provide superbills upon request. Prospective clients are encouraged to contact their insurance provider to determine what out-of-network benefits they may have.

I accept cash, check, and all major credit cards. Fees are due at the time of service.

Please cancel appointments at least 48 hours in advance to avoid incurring a late fee.
